• Complete an annual wellness check-up with a primary care provider (PCP) – 250 reward points ($25 value). *
• Complete your child’s immunization vaccines by the 13th birthday – 600 reward points ($60 value). *
• Members with hypertension for completing a blood pressure test showing that their blood pressure is under control can earn- 250 reward points ($25 value). *
• Women ages 50-74 who complete a mammogram can earn – 250 reward points ($25 value). *
• Women ages 21-64, who complete cervical cancer screening can earn – 250 reward points ($25 value). *
• Members 0-21 who completes Dental check-up – 200 reward points ($20 value). *
• Complete a full childhood immunization series- earn up to 300 reward points ($30 value). *
• Members with type 1 or type 2 diabetes for completing a blood pressure test showing that their blood pressure is under control can earn – 250 reward points ($25 value). *
• Complete a follow-up outpatient visit with a mental health provider within 7 days after discharge from the hospital for a mental health condition can earn – 300 reward points ($30 value). *

How to Earn Points

You automatically earn rewards when you complete your health care visits for preventive care, check-ups, or health screenings. You need not make any extra effort to earn your points, Aetna gifts you for every healthy step you take. Register your account and activate your card on the Aetna website. Once your account is set up, you will accumulate points for the necessary health steps.
